CDC: Foodborne Illness in the U.S. Not Getting Better

The Centers for Disease Control and Prevention today released their annual survey of foodborne illnesses in the United States, and the news is, well, not great. In the words of the press announcement they sent out to announce the data release: “limited progress.”

The survey — technically the Foodborne Diseases Active Surveillance Network, but usually known as FoodNet — doesn’t cover the entire US; it’s a representative sample drawn from 10 sites in nine states where the CDC already has arrangements with epidemiologists and laboratory personnel. Those 10 sites, most of them at state health departments, cover 48 million people, or about 15 percent of the US population. So among that slice, in 2013, there were:

  • 19,056 lab-confirmed foodborne illnesses,
  • 4,200 of which were severe enough to cause the person to be hospitalized,
  • and 80 of which caused the person’s death.

(For context, the CDC’s extrapolation of foodborne illness nationwide, made in 2011, was 48 million illnesses, 128,000 hospitalizations and 3,000 deaths.)

The agency compared the 2013 numbers against two sets of data, one set taken covering 2010-12 and the other 2006-08. Its summation, from its report in its weekly publication MMWR:

Compared with 2010–2012, the estimated incidence of infection in 2013 was lower for Salmonella, higher for Vibrio, and unchanged overall. Since 2006–2008, the overall incidence has not changed significantly. More needs to be done.

There's a Major Foodborne Illness Outbreak and the Government's Shut Down

Late-breaking news, and I’ll update as I find out more: While the government is shut down, with food-safety personnel and disease detectives sent home and forbidden to work, a major foodborne-illness outbreak has begun. This evening, the Food Safety and Inspection Service of the US Department of Agriculture announced that “an estimated 278 illnesses … reported in 18 states” have been caused by chicken contaminated with Salmonella Heidelberg and possibly produced by the firm Foster Farms.

“FSIS is unable to link the illnesses to a specific product and a specific production period,” the agency said in an emailed alert. “The outbreak is continuing.”

A Few Ways the Government Shutdown Could Harm Your Health (And the World's)

There’s going to be a lot — a lot — of coverage today on the federal shutdown, what it means and how long it might go on. I thought it might be worth quickly highlighting how it affects the parts of the government that readers here care most about: public health, global health, food safety and the spread of scary diseases.

Most of those government functions are contained within the Cabinet-level Department of Health and Human Services, where 52 percent of the employees have been sent home. So the news is not good.

USDA: Chicken Processed in China Can be Sold in the US Without Labels to Say So

Catching up to this news, which dropped quietly just before the holiday weekend: In a first, the US Department of Agriculture has given permission for chicken products processed in the People’s Republic of China to be sold in the United States without labeling that would indicate where the chicken products came from.

The news was broken by Politico, whose writers obtained USDA documents before the agency released them, and then followed up by the New York Times, with some no-holds-barred analysis by Bloomberg Businessweek.

If you’ve been reading for a while, you’ll know that food safety in China is well below US standards. (See this post for stories of toxic vinegar, glow-in-the-dark pork, and more.) So it may be a surprise to hear that birds grown and slaughtered outside that country, but cooked and made into products in it, would be acceptable for sale here. Especially since the plants that USDA has approved for sales into the US market will not have USDA inspectors on site.

Advice for the Annual Observance of Food-Poisoning, Umm, Thanksgiving Day

My grandparents — children of Irish and Scottish immigrants, for whom calories per penny was a much more important food value than fine cuisine — had a little mnemonic for Thanksgiving. It went like this:

Turkey, tetrazzini, ptomaine.

Perhaps that requires a little explanation.

The turkey part should be self-evident. Tetrazzini — a cream-sauce casserole based on spaghetti, one of those early 20th-century dishes invented to honor Italian opera stars — was what they did the second day with the turkey leftovers. Ptomaine (the “p” is silent) was what they worried lay in wait for them on the third. A late 19th-century term that has passed out of use, it derived from the notion that poisonous compounds lurked in rotting food.

For people who grew up before the antibiotic era — and who learned to cook when refrigerators were literal ice chests that kept things cool at best — “food poisoning” was a reasonable fear, and a risk they refused to take. On the Saturday after Thanksgiving, no matter how delicious it appeared, whatever remained of the turkey went into the trash.

People Want to Eat Meat Raised Without Excessive Antibiotics. Wouldn't You?

This news is going to be everywhere today, but it’s solidly in the topics I care about (and you readers care about — at least I think you do), so I’m going to cover it regardless.

The magazine Consumer Reports is publishing a report and poll on US consumers’ attitudes toward the overuse of antibiotics in agriculture. From everyone’s reactions when I write about this, I thought people cared about this issue, but the numbers are a little surprising even to me: 86 percent of shoppers in a nationally representative sample of 1,000 adults said they wanted meat raised without antibiotics to be available in their local supermarkets. High Levels of Resistant Bacteria on Meat (Again)

A new report is out from the federal collaboration that monitors antibiotic resistance in animals, retail meat and people, and the news is not good.

The full title is the 2010 Retail Meat Report from the National Antimicrobial Resistance Monitoring System. This report is issued by the Food and Drug Administration; the humans one comes from the Centers for Disease Control and Prevention and the animals one from the US Department of Agriculture. It reports the results of testing on 5,280 meat samples collected in 2010 in California, Colorado, Connecticut, Georgia, Maryland, Minnesota, New Mexico, New York, Oregon, Tennessee, and Pennsylvania. (Those are sites of state labs participating in a federal surveillance network, FoodNet, plus one volunteer lab, Maryland.)

The report — which is broken down first by foodborne organism and then by meat type — notes a number of instances where either the percentage of bacteria that are antibiotic resistant, or the complexity of the resistance, is rising. Quoting from the report:

More MRSA Found In U.S. Retail Meat (Turkey, Too)

There are two new studies out that confirm, once again, that drug-resistant staph or MRSA — normally thought of as a problem in hospitals and out in everyday life, in schoolkids, sports teams, jails and gyms — is showing up in animals and in the meat those animals become.

The strain of staph that shows up in farm animals, known as “livestock-associated” MRSA or MRSA ST398, first emerged in pigs in the Netherlands, and has been widely identified in retail meat in Europe. (You can find a long archive of posts on ST398 here, and more here.) But that same strain has been difficult to identify in the United States. That may be due in part to the U.S. having a uniquely massive epidemic of community-associated MRSA, far larger than in any other country, which likely both obscures any animal epidemic from detection, and possibly also fills the ecological niche that livestock-associated staph might otherwise occupy. But, it must be said, there’s also remarkably little political will to look for livestock-associated MRSA (though the U.S. Department of Agriculture has now funded a study).

Nevertheless, individual investigators keep trying, and when they look for the pathogen, they find it, such as in these studies from May and this one from April. Now, two other teams have as well.

Big News But: USDA Bans "Other" E. coli Strains

Good news, but not excellent news, today from the US Department of Agriculture: It has agreed that, starting in March 2012, six more strains of E. coli will be considered “adulterants,” putting them in the same regulatory category as the much-feared E. coli O157:H7.

It is a big step, to do this. Those six bacterial bad actors — technically, E. coli O26, O45, O103, O111, O121, and O145 — are now responsible for the majority of foodborne illness caused by E.coli in the United States, causing almost twice as much illness each year as O157 does.

The biggest foodborne-disease threat may not be addressed by the new food-safety law

So if we wanted to reduce the danger of pathogens passing to people via food — not just drug-resistant bacteria, which are an increasingly significant problem, but all disease-causing ones — where to start?

Formulating a strategy is more difficult than it seems. In the US, policing food safety is divided among several federal agencies: the FDA, USDA and CDC. The FDA has responsibility for most of the food supply, including seafood, produce, processed food and fresh eggs. The USDA’s Food Safety and Inspection Service (FSIS) regulates fresh meat and poultry and egg products. The CDC surveys the illnesses that result from any of them, estimating most recently that one in six US residents, or about 48 million people, get sick each year, 128,000 are hospitalized and 3,000 die.

But most of those illnesses are never investigated, because a substantial portion of them occur individually or in small clusters, not in major outbreaks. Many of them have long-term consequences that are never recorded by any federal counting mechanism. And there’s currently no surveillance system that links pathogens and food — which means there’s no way to target which foods, or food-raising practices, pose the greatest risks.

The new food-safety bill, signed in January, addresses at least some of those barriers, by requiring a risk-based approach to foodborne illness — meaning, you look at what is causing the greatest problem, and aim your efforts and funding in that direction. But the bill — which certain Congressmen have threatened to starve of funding — covers primarily the FDA. And a new analysis suggests that’s not where the greatest problems lie. [Read more…]
