Breaking News: Judge Orders FDA to Examine Safety of Ag Drugs

Here’s breaking news that I’ve learned about. The significance is going to unfold over the next 24 hours and more, and when it does, I will update. (Update: Overnight, a lot came out about this, so I’ve wrapped it all into a new post here.)

Here are the basics:

A district judge in New York has ruled in favor of a coalition that sued the Food and Drug Administration in order to compel the agency to follow through on its 35-year-old attempt to exert control over antibiotics in animal feed known as “growth promoters” — tiny doses of antibiotics delivered in feed and water that are believed to stimulate the evolution of antibiotic-resistant organisms.

Technically, Magistrate Judge Theodore H. Katz has granted the groups’ motion for summary judgement, and denied the FDA’s request that the suit be dismissed.

The decision, filed this afternoon, does not compel the FDA to ban growth-promoter (or “feed efficiency”) use of antibiotics. What it does appear to do, though, is require the FDA to follow through on a process that it began in 1977, when the agency was so concerned over the safety of using penicillin and tetracycline drugs in livestock feed that it called hearings to examine withdrawing its approval of using the drugs in animals.

Because of pressure, largely from certain Congressmen, those hearings were never held.

Bats, Booze, Bugs, Birds, Blood and Bushmeat (ICEID 4)

Since I started electing to do blog coverage of scientific meetings, I’ve run into an unfortunate reality. On any meeting day, there are one or two presentations that either are strikingly newsworthy or fit into an ongoing topic that I’m already interested in, and that therefore I feel obliged to write about. That means I’m unable to cover dozens, sometimes hundreds, of other interesting papers and posters.

I feel bad about this, especially when authors stop what they are doing to talk to me. So here’s my admittedly insufficient remedy: a quick round-up of a few of the hundreds of intriguing presentations this past week at the biennial International Conference on Emerging Infectious Diseases. Gastroenteritis Deaths, Foodborne Outbreaks Increase (ICEID 3)

Several related pieces of news today from the International Conference on Emerging Infectious Diseases:

  • The number of people who are dying from illnesses that involve vomiting and diarrhea more than doubled between 1999 and 2007, and most of the increase was due to Clostridium difficile.
  • Disease outbreaks caused by imported foods are rising, and tainted foods are coming into the United States from a wider array of countries than before.

Acorns And Mice Driving Unusual Lyme Disease Risks (ICEID 2)

The northeastern United States faces potentially “the worst year yet” for Lyme disease and other tickborne infections because of the periodic abundance of a little-noticed component of the disease’s complex ecology: acorns.

Dr. Richard S. Ostfeld of the Cary Institute of Ecosystem Studies in Millbrook, New York, explained during a presentation Tuesday at the International Conference on Emerging Infectious Diseases (ICEID) that a heavy crop of acorns in 2010 — a phenomenon known as a “mast year” — fueled a 2011 population bloom in white-footed mice, which stash acorns for winter food and begin breeding earlier in years when they are well-fed. That surge intersected with the two-year lifecycle of the ticks that transmit Lyme disease, for which mice are the key host, and this summer could produce a bumper crop of infected tick nymphs looking to bite large mammals — including humans.

“We’re already working with health departments” in Lyme-endemic areas to help craft messages to the public about the potential risk, Ostfeld said during his talk.

The prediction, which is based on earlier work by Ostfeld and colleagues (including these papers in 2006, 2005 and 2001) relies on the key role that white-footed mice (Peromyscus leucopus) play in perpetuating Lyme disease. That species, he said, appears to be the most competent reservoir for Borrelia burgdorferi, the bacterial cause of Lyme. Mice sustain the infection without cost to themselves, are frequently bitten by tick larvae, and groom off or otherwise kill the larvae at lesser rates than other small mammals that are bitten — allowing the larvae to drop off naturally and complete their transformation into tick nymphs that transmit infection in their second year of life.

Mice also can survive in much smaller areas than the larger animals, chiefly deer, that are usually blamed for perpetuating Lyme, Ostfeld pointed out. In sampling of “forest fragments” sliced up by development in three northeastern states, his team has not found a parcel in which mice did not thrive. Larger parcels with more balanced ecosystems, with natural mouse predators and larger mammals, actually tend to have lower Lyme density, he said.

Because of the yearlong gaps between bumper crops of acorns, mice, and then ticks, the reliable but irregular masting phenomenon could be used as an early-warning signal for Lyme exposure risks, Ostfeld pointed out. Oak trees mast roughly every three to five years, “and when you are in a mast year, you always know it,” he said.

Staph In Pigs And Pig Farmers: The Latest Reports (ICEID 1)

I’m spending the first part of this week at the International Conference for Emerging Infectious Diseases, a biennial scary-disease nerdgasm that is sponsored by the Centers for Disease Control and Prevention, the American Society for Microbiology and other worthy organizations, and in this iteration has drawn about 1,600 participants from more than 50 countries. (You can see the program here. I had planned to live-blog it, but the hotel was just renovated and apparently they built a Faraday cage into it, since connectivity is a painful trickle.)

On Monday’s agenda, among other intriguing talks: Two updates on MRSA, methicillin-resistant S. aureus, among farmers in two states.

Flu Infections and MRSA Deaths in Maryland

Sad news out of Maryland, and a reminder of how devastating MRSA, methicillin-resistant Staphylococcus aureus, can be when it combines with flu infection. According to the Maryland Department of Health and Mental Hygiene, the Washington Post and ProMED, five members of a family have fallen ill and three have died from MRSA pneumonia that took hold in lungs inflamed by flu infection.

The dead are Ruth Blake, 81, and her children Lowell, 58, and Vanessa, 56. Another child, Elaine, also fell ill and was hospitalized, and Ruth Blake’s sister has been hospitalized also. They had all contracted one of the seasonal flu strains circulating this year: H3N2. According to the Post, Ruth Blake was vaccinated against flu this season; her children were not. The assumption is that both flu and MRSA spread from the mother to the children.

High Levels of Resistant Bacteria on Meat (Again)

A new report is out from the federal collaboration that monitors antibiotic resistance in animals, retail meat and people, and the news is not good.

The full title is the 2010 Retail Meat Report from the National Antimicrobial Resistance Monitoring System. This report is issued by the Food and Drug Administration; the humans one comes from the Centers for Disease Control and Prevention and the animals one from the US Department of Agriculture. It reports the results of testing on 5,280 meat samples collected in 2010 in California, Colorado, Connecticut, Georgia, Maryland, Minnesota, New Mexico, New York, Oregon, Tennessee, and Pennsylvania. (Those are sites of state labs participating in a federal surveillance network, FoodNet, plus one volunteer lab, Maryland.)

The report — which is broken down first by foodborne organism and then by meat type — notes a number of instances where either the percentage of bacteria that are antibiotic resistant, or the complexity of the resistance, is rising. Quoting from the report:

Also Receiving Antibiotics on Factory Farms: Shrimp

Tom Philpott of Mother Jones had a great post earlier this week looking at the vast environmental damage caused by shrimp farming in South and Southeast Asia. He takes off from a presentation at the annual meeting of the American Association for the Advancement of Science by J. Boone Kauffman of Oregon State University, which examined the destruction of coastal mangrove forests that allows shrimp farms to be established and found that shrimp’s carbon footprint is 10 times higher than that of beef cattle. 'Pig MRSA' Came From Humans, Evolved Via Farm Drugs

In the summer of 2004, a 6-month-old girl who lived in the southeastern part of the Netherlands — prime, intensive hog-farming country — went in for surgery for a birth defect of her heart. As is routine in the Netherlands, which has excellent hospital infection control, she was checked before surgery for MRSA, the drug-resistant bacterium that can live on the skin without causing infections and can be unwittingly transmitted from one patient to another. The girl was carrying MRSA, which was a surprise — but the bigger surprise was that her MRSA strain did not render any results on the standard identification test, PFGE.

Looking for a source for the mysterious strain, the hospital epidemiologists where the girl was being treated asked to check her family: father, mother, school-age sibling. They were carrying it. They asked to check the family’s social circle; some of them were carrying it too. Then, flailing about for an answer — the Netherlands has such low rates of MRSA that these persistent findings were really rather odd — the epidemiologists asked what the family and their friends all did for a living, and received the answer that they were all pig farmers. So they checked the pigs, and the pigs were carrying the MRSA strain as well. And if a new MRSA strain in humans was odd, then a MRSA strain in pigs was very odd — because swine have their own varieties of staph, and are not supposed to get S. aureus, the usually human strain that accounts for the “SA” in MRSA.

That summer of detective work (which is told in full in my book Superbug) provided the first sighting of what would come to be called MRSA ST398, or in Europe CC398: a strain which was not quite like hospital MRSA, and not quite like community MRSA, and which carried a distinctive signature of resistance to tetracycline, a drug that is not much used for human MRSA but is routinely used in confinement-style farming. From its first identification, ST398 spread rapidly through Europe, and then into Canada, and then to the United States, being found first in pigs and pig-farm workers, and then in retail meat, and then in people with no connection to farming at all.

The only mystery was where it had come from.

Highest Rates Ever Recorded of Multi-Drug-Resistant TB

On the heels of the news of totally drug-resistant (TDR) TB being identified in India — and disavowed, unfortunately, by the Indian government — the World Health Organization has released an update on the background situation of drug-resistant TB around the world.

The news is not good. Drug-resistant TB is at the highest rates ever recorded.

What Bacteria Do, And How Health Insurance Won't Help

I spend a lot of time here talking about the damage that bacteria can do, and how sick they can make you. I don’t spend much time talking about their collateral damage: the hospital stays, the unbelievable costs of care, the bewildering complexity of the American healthcare and insurance systems, and the disillusionment that encountering both of those leaves behind.

To rectify that, I’d like to introduce you to my acquaintance Kevin Zelnio.

It wouldn’t be fair for this story to claim that Kevin and I are friends. We’re part of the same crowd in the science blogosphere and in the real-life annual bloggers’ meeting called Science Online. I like him and I’m impressed by his spirit and talent, but I can’t say that we’re close — which is just to say, this post isn’t written with any inside knowledge.

Moving Beyond Factory Farms: Pastured Poultry

It’s astonishing what you can not know about the place where you live. Me, for instance: I live mostly in Atlanta. I’ve lived here twice, once for 10 years as a newspaper reporter, and now — after a four-year break — for 15 months so far as a writer (and trailing spouse, which is how I got back here). Between working for one of the (formerly) largest papers in the country, liking long drives, and regularly indulging my ungovernable curiosity, I thought I knew Georgia pretty well.

I was wrong. Here’s what I learned about Georgia on Thursday:

  • It raises more meat chickens than any other place in the United States, about 1.4 billion of them a year.
  • That’s 15 percent of all the animals raised in confinement agriculture in the United States. Not just 15 percent of the chickens; 15 percent of everything.
  • All those chickens produce 2 million tons of poultry manure and litter a year, one-fifth of what the entire U.S. poultry sector produces.
  • That waste is applied on land — including land where other food crops are grown — from which it can run off and contaminate water supplies.
  • 40 to 80 percent of gut bacteria recovered from confinement chicken-houses are multi-drug resistant.
  • Caring for foodborne illness from organisms carried on chicken, and making up for lost productivity when people are made sick, costs about $2.4 billion per year.
  • Chicken catchers, who cage the birds on their way to slaughter, may lift 5,000 pounds in an hour. Slaughterhouse line workers may perform the same repetitive cutting motions 20,000 to 30,000 times in a work shift.
  • Slaughterhouses in Georgia kill 1 million chickens per week.
  • Poultry is exempt from humane slaughter regulations.

Goodbye, Magic Bullet: Hello, Highly Resistant Gonorrhea

One hundred years and some months ago — on March 16, 1911 — a publication called the Boston Medical and Surgical Journal printed a description of the treatment of 20 men at Walter Reed General Hospital in Washington, D.C. The men were members of the U.S. Army. Because medical privacy rules were a lot looser 100 years ago, we know most of their names, because the journal gave them: Cornwell, 22 years old; Smith, 24; Randall, 29; Simerly, 27; Brown, 23; Davis, 26; Waits, 22; Allen, 19; Kimmitt, 27; Megee, 29; Laird, 40; Brinson, 34; Franklin, 29; Clements, 34; Johnson, 24; Lorah, 23; and Crabbe, 27.

The men were in Walter Reed, which we now know as Walter Reed Medical Center, because they all had the sexually transmitted disease syphilis. A century later, it seems absurd to be hospitalized for an STD — but in the years before antibiotics, syphilis and gonorrhea were fantastically destructive of productivity. The men’s records, excerpted by the journal, capture that: They had arthritis, ulcerated skin, heart-valve failure, necrosis of the skull. Cornwell had been “on sick report” — unable to report to duty — for 4 months and 10 days; Simerly, for 5 months and 23 days; Kimmitt, for 11 months; Johnson, for 1 year.

Fast-Spreading Animal Virus Leaps Europe, UK Borders

A newly identified disease is moving rapidly through livestock in Europe and has authorities both worried and puzzled. The disease, dubbed Schmallenberg virus for a town in west-central Germany where one of the first outbreaks occurred, makes adult animals only mildly ill, but causes lambs, kids and calves to be born dead or deformed.

The United Kingdom’s Animal Health and Veterinary Laboratories Agency (AVHLA) said today that the virus has been found on 29 farms in England; in the past few weeks they found it in sheep, but today announced that they have identified it in cattle as well. In mainland Europe, it has been identified on several hundred farms in the Netherlands, Germany and Belgium, and most recently in France. The European Center for Disease Prevention and Control has said that the new virus’s closest relatives do not cause disease in humans — but that other more distantly related viruses do:

The new virus belongs to the Bunyaviridae family, genus Orthobunyavirus, Simbu serogroup (preliminary information, based solely on genetic information)… Genetic characterisation has shown that the new virus is closest to the following Simbu serogroup viruses: Shamonda-, Aino- and Akabane-viruses, which do not cause disease in humans.
However, at least 30 orthobunyaviruses are zoonotic and may cause disease in humans, with symptoms ranging from mild to severe — e.g. La Crosse encephalitis virus, California Encephalitis virus, Cache Valley virus, Batai virus, Tahyna virus, Inkoo virus, Snowshoe Hare virus, Iquitos virus and Oropouche virus.

What Would It Take to Make a MRSA Vaccine?

If you accept that MRSA, drug-resistant staph, kills up to 19,000 people per year in its most virulent forms, and causes at least 7 million primary-care and ER visits per year — and there’s no reason to doubt either of those estimates, which were published in the Journal of the American Medical Association and the Archives of Internal Medicine — then it’s reasonable to ask: Why can’t we prevent it?

I wrote a book on MRSA, immersing myself for several years in thousands of medical-journal articles and hundreds of patients’ stories, and when I surfaced from all that, I had the same question. Why don’t we have a vaccine against MRSA? How hard could it be?

The answer, as it turns out, is: surprisingly, heartbreakingly hard.

Drug Resistance in Pork: More Going On Than Appears

A paper released recently, by the University of Iowa team that is the lone US research group tracking “pig MRSA” ST398, caused a ripple. (It came out while I was at ScienceOnline and it’s taken me a while to catch up.) The paper compares the occurrence of MRSA, drug-resistant staph, on various cuts of retail pork from pigs that were raised either conventionally in confinement, with routine use of antibiotics, or in an alternative rearing scheme with no antibiotics. (NB: Not “organic,” despite what some headlines said; that’s a separate issue of USDA licensure.) The team found that both conventionally raised and antibiotic-free meat carried MRSA, both the human-associated kind and the pig-adapted kind.

The TL;DR over the past week has been: There’s just as much resistant bacteria on drug-free meat as there is on conventional meat, so why spend the money — or raise the alarm over farm antibiotic use?

My interpretation is a little more nuanced. But my takeaway is that, in its underlying data, the study proves what campaigners against ag antibiotic use keep saying: that once you use antibiotics indiscriminately and drive the emergence of resistant organisms, you have no way of predicting where that resistance DNA will end up.

TDR-TB: The Indian Government Denies It

An update to the news two weeks ago of totally drug-resistant tuberculosis, TDR-TB, being identified in India (and earlier in Italy and Iran): The Indian government has announced that it doesn’t exist, and is putting pressure on the physicians who identified it to say they made a mistake.

Because, of course, that’s going to keep a disease from spreading.

Why Is Type 1 Diabetes Rising Worldwide?

We’ve gotten sadly accustomed by now to warnings about obesity and its effect on health: joint damage, heart disease, stroke, diabetes and its complications such as blindness and amputation. We almost take for granted that as obesity increases worldwide, diabetes will also, and it is. That is, type 2 diabetes — the kind that is linked to obesity and used to be called adult-onset diabetes — is rising as obesity does.

But here’s a puzzle: Type 1 diabetes — the autoimmune disease that begins in childhood and used to be called juvenile-onset diabetes — is rising too, around the globe, at 3 percent to 5 percent per year. And at this point, no one can quite say why.
